The Role of Pharmacists in the Care Coordination Puzzle

June 10, 2014 - Curant Health Director of Clinical Services, Vickie Andros, in FierceHealthcare - recently contributed to an article published in FierceHealthcare about collaboration between pharmacists and health specialists as the need for comprehensive healthcare continues to escalate. Vickie tells Katie Sullivan of FierceHealthcare that “Medication management, medication reconciliation, and education and behavior counseling are now a more integrated, collaborative effort between pharmacists, doctors and patient navigators.” This group effort is necessary in the healthcare field, especially for patients with a chronic illness that require several different medications.

Perhaps more than ever, pharmacists and other specialists play more of a role in direct care thanks to care coordination efforts.

Medication management, medication reconciliation, and education and behavior counseling are now a more integrated, collaborative effort between pharmacists, doctors and patient navigators, according to Vickie Andros, director of clinical services at Curant Health, a chronic disease management healthcare company, in an exclusive interview with FierceHealthcare. Her team of pharmacists work directly with patients on medication discovery and medication reconciliation.
